You can modify or update the values in the spreadsheet as needed.

• They can be completed digitally using Adobe Reader.
The forms are intentionally designed without dropdown menus so they remain clean and printable for manual completion if preferred.
Do I need a license to operate a non-medical home care agency?
Licensing requirements vary by state or province.
In most locations you will need to register your business, but some states also require a home care agency license before services can begin.
We recommend contacting your local Department of Health or regulatory authority to confirm the requirements for your specific location.
You can also review the state-by-state startup guides available on our website to better understand licensing expectations.
Can I resell or redistribute the templates?

They may not be:

Templates are licensed for individual business use only.
